Terms and Agreement

Family Plan Terms:

  • Plan covers 2-4 family members living in the same household
  • Children must be under 26 or dependents of the primary account holder
  • Plan membership is non-transferable and valid for one year from enrollment
  • All family members must receive care at Copley Dental to maintain plan benefits
  • Primary account holder is responsible for all plan payments and scheduling
  • Cancellation requires 30-day notice; partial refunds based on services used
